Emma's 9th Birthday

I as the parent/guardian of , do hereby give permission for my child to participate in party and/or decorating activities at Dolce Vita Desserts. I agree that I will not hold Dolce Vita Desserts, or its employees, liable neither in case of accident, injury, nor in case of lost or damaged personal property.

Dolce Vita Desserts is not an allergen free kitchen. Therefore, Dolce Vita Desserts cannot be held liable for allergic reactions during or after the party. I understand that it is my responsibility to inform Dolce Vita Desserts of any allergies. I also understand that it is my responsibility to assess the risk of exposure based on the severity of the allergy.

If I cannot be contacted, I give permission to authorize any emergency medical care that may appear necessary. In addition, if I cannot be contacted within 20 minutes after a birthday party, I grant permission to the host parent to arrange a ride for my child.

 I have read and understand the above. I hereby grant permission for my child to participate in decorating activities and consumption of products at Dolce Vita Desserts.

I grant to Dolce Vita Desserts, its representatives and employees the right to take photographs of my child during the party. I agree that Dolce Vita Desserts may use such photographs of my child without his/her name and for any lawful purpose, including publicity, illustration, advertising, and Web content.

Dolce Vita Parties and Classes are drop and go events. You may arrive 10 minutes before the end of the event for picture time and pick-up. It is necessary that all information be completed and returned to the host parent or emailed to orders@dvdesserts.com in order for your child to participate.
